Act of Death

To have a death registered with the Consular Office of the Embassy of Greece in Tokyo, the deceased must have passed away in the Consular Office’s jurisdiction and submit the following documents:

1. Application to the Consular Office by a relative or a witness,
2. Death Certificate, where cause of death is called signed by a doctor,
3. Greek Passport of the deceased (if available),
4. Greek ID of the deceased (if available),
5. Certificate of municipal registration from the appropriate municipality in Greece.

The fee for an act of death is €1.00 (Presidential Decree 116, 30.04.2006, Government Gazette 96, Volume I). The cost for each copy is €1.00. There will be 2 copies required therefore the total cost will be €3.00.
